Prior to filling out this Electronic Application please ensure you are in possession of all the below requirements. Once the application is started, it must be completed as it will not save. A digital copy (upload file directly via the form) of all the below requirements will be accept as means of verification. The AAOMPS Board of Directors reserves the right to request any and all original documents at any time. Should the AAOMPS Board require either additional information or verification, the applicant will be notified via the email provided. All applicants are required to maintain a physical copy of all application records.

AOMPEB National Board Exam
Proof of Passing Marks
Electronic copy of Official Transcripts
UCOMPE accredited educational institute listing the degree Conferred
Maintain and provide proof of current CPR** certification
**US Based Practitioners (Basic Life Support, Professional rescuer, or any other advanced distinction)
2 Letters of Recommendation
Submit a current passport size headshot/photo
Must be within the past 6 months
Submit a copy of a government issued photo identification
Driver's License or Passport
Maintain and provide proof of current professional liability insurance
($2,000,000 minimum coverage)
Full Disclosure of any criminal convictions or prior professional misconduct
Be able to speak and write English with reasonable fluency
